- Emergency telephone calls should be placed only if you or your child require(s) a potential Emergency Room evaluation.
- Urgent telephone calls are for medical concerns that cannot wait until the office opens the next day.
- Non-urgent medical issues, routine questions and concerns, medication refills and scheduling of appointments are best addressed during regular office hours.
Instructions for contacting the on call physician for emergency and/or urgent medical issues are available by calling our main number 410-321-9393.
In the unlikely event that our phone system malfunctions or you do not receive a timely response, please go directly to the nearest emergency room.

Financial Policy Disclosure and Agreement

These policies protect our ability to successfully provide care and responsibly adhere to mandated guidelines established by patient selected and contracted insurance companies. Appointments
Patients are seen in the office by appointment only. For your initial visit please arrive in the office fifteen minutes early to fill out necessary paperwork. If you prefer, we can fax or email the forms to you in advance or you can download them from this site. Please bring your child's insurance card to all appointments. Your cooperation is necessary to keep us on schedule.
Future Appointments
Well Child Appointments are the routine visits scheduled at regular intervals with your child's pediatrician. These visits usually include a history, physical examination, and time for discussion of problems. Immunizations, routine blood, urine and tuberculin tests, vision and hearing screening are all done at these visits as indicated. After your newborn's initial office visits, infants are seen every 2-3 months for the first 12 months. During the second year, children are seen at 15 months, 18 months and 2 years. Children are seen annually thereafter. These appointments are typically scheduled at least two to three months in advance. Please call by May to schedule visits needed over the summer.
School and Sports physicals are often required by schools within the six month period before school begins. If you know that your child will require a school or sports physical please call ASAP to schedule. During the summer months, the pediatric nurse practitioners offer additional Saturday sports physical clinics. Spaces in these clinics are limited so please schedule early.
Follow up appointments are appointments that your pediatrician has requested you make to evaluate ongoing medical issues. Examples of these appointments would be weight check visits, headache follow up visits, developmental follow ups visits and asthma follow up.

Acute Sick Appointments are appointments needed for a new medical concern. Examples of these appointments would be evaluation of ear pain, sore throat, cough, wheezing, rash and abdominal pain. We will do our best to schedule these appointments with your primary pediatrician but you may also be offered an appointment with another physician or pediatric nurse practitioner in the office. Since these appointments are for acute issues, your child's appointment will be scheduled for the first available appointment that day in order to accommodate all children who need to be seen.

As a convenience to our patients, we offer an EARLY morning call time for same day, quick sick appointments. Monday - Saturday Please call 410-427-1690 from 7:30 am - 8:15 am.
Please note: Acute Sick Appointments may be not scheduled via email.
Missed Appointments/Late Cancellations
Broken appointments represent a cost to us, to you and to other patients who could have been seen in the time set aside for you. Cancellations are requested 24 hours prior to the appointment. We reserve the right to charge for missed or appointments not cancelled within 24 hours appointments. The charge for missed appointments is $25. Excessive abuse of scheduled appointments may result in discharge from the practice.

Insurance & Payments
We contract with many insurance plans and accept cash, check, Mastercard and Visa.
Payment of all Co-Pays and Deductibles must be made at the time of all visits. You will be expected to show your insurance card each and every visit. If your child comes to the office alone or is brought in by someone other than a parent, please make sure your child's insurance card or a copy accompanies him/her. If you do not have your insurance card with you at the time of your child's visit, you may be asked to sign a waiver for services incurred since we will not be able to verify your insurance.
If we do not participate with your insurance company, full payment is expected at the time of any office visit.

Referrals
If your insurance requires a referral authorization, you must obtain that authorization from our primary physicians prior to going to your appointment except in emergencies.
Please call our referral coordinator (410-427-1648) at least seven business days in advance to allow us to assure that your documentation is complete. Emergencies will be handled accordingly.

Record Requests
If you require a copy of any portion of your child's record please contact us at 410-321-9393. Please note that we require at least 7-14 days for completion of these requests. There is a fee for each page you request from your child's or your medical record. If you require a copy of the records by the end of the next business day, we would be happy to compile those records for you for an additional fee of $25. All record requests must be accompanied by a completed
record request form.

Completion of Medical Forms
Because of the increasing demand, the complexity and repetition of the forms as well as the length of time it takes to complete forms, it is imperative that you bring your form to us as soon as it is received. The usual turn around time to complete, review and sign a form is a minimum of 7 to 10 business days.
There is a charge for completion of health forms on a per form basis- simple form $10, standard form $15, emergency form (one that is needed with 24 hours)- $30, complex form (the charge will be determined by the physician based on the complexity of the form). Please include a stamped and addressed envelope if you will not be picking up the completed form.
