Transferring Patients

We are accepting new patients and we would be delighted to welcome your family to MPG.
 
To join the practice, please:

1. Verify that your insurance is accepted here.
2. Review our vaccination policy here.
3. Select your top choice of three providers here or you can request “any provider.” 
4. Fill out our new patient intake form here. A staff member will call you within 1-2 business days to complete the intake process and schedule your appointment.
5. If you prefer to talk to someone, call 410-321-9393 to complete the new patient intake process via phone. (follow prompts to the provider at the top of your list, or prompt to POD M if you are comfortable with “any provider.”)
6. Contact your current pediatrician and obtain your child’s records, especially growth chart and immunization records. Make a copy for us, and keep one for yourself. We will need this prior to your child’s first visit at our office.
7. Once you have been accepted into a specific provider’s panel AND your records have been received, you will be notified and you can schedule your first appointment.
